Screening Questions

  1. Have you come into close contact with anyone with acute respiratory Illness or travelled outside of Ontario in the past 14 days?
  2. Have you tested positive for Covid-19 or come into close contact with a confirmed or potentially positive case of COVID-19?
  3. Do you have any of the following symptoms:
    • Fever
    • New onset of cough
    • Worsening chronic cough
    • Shortness of breath
    • Difficulty breathing
    • Sore throat
    • Difficulty swallowing
    • Decrease or loss of sense of taste or smell
    • Chills
    • Headaches
    • Unexplained fatigue / malaise / muscle aches (myalgias)
    • Nausea/vomiting, diarrhea, abdominal pain
    • Pink eye (conjunctivitis)
    • Runny nose/nasal congestion without other known cause
  4. Are you a frontline/ essential worker?

Do Physio COVID-19 Waiver Form

Protocols for entering/while on the Do Physio premises

  1. Text once you have arrived but do not enter until instructed to do so. Please enter from the outside terrace entrance and not via the entrance within the building.
  2. Everyone entering Do Physio, must wear a mask and or shield otherwise entry will not be permitted. It can be a homemade mask, or even a simple scarf/ bandana covering the nose and mouth.
  3. Remove footwear before entering.
  4. Sanitize hands upon entering.
  5. Proceed to bathroom and change into a clean change of clothes including socks and
    place your outdoor clothes in the plastic basket or hanger provided.
  6. Wash your hands with soap for at least 20 seconds then proceed to treatment room.
  7. Payments via ​contactless​ debit/credit card or e-transfers only. No cheques or cash payments will be accepted at this time.
  8. Sanitize your hands upon leaving.

Do Physio’s protocol/responsibility

  1. Disinfecting the treatment room, bathroom and any items used or touched after each client.
  2. Washing hands and arms above the elbow before each client.
  3. Wearing a clean gown and a mask (with filter), which will be changed after each client.
  4. Wearing a shield which will be disinfected after each client.
  5. A medical grade air purifier will be running at all times while the office is open.
  6. Thorough cleaning and disinfecting of the office at the end of each day.
